Speaker Release Form.

The purpose of this form is to gather relevant data that will be used to promote female blockchain speakers on the TokenWoman website.

Please note that all of the data captured in this survey will be processed by TokenWoman (in the following also referred to as “TokenWoman”, “our”, “we” or “us”).

The legal basis for our data processing is Art. 6 Par. 1 S.1 lit a Basic Data Protection Ordinance (“DSGVO”), as enacted by the European Union. Our legitimate interest follows from the purposes listed above for data collection.

We only collect, use and pass on your personal data if it is permitted by law (in particular by the DSGVO) or if you consent to the collection of your personal data. Personal data is hereby defined as all information that serves to determine your person and which can be traced back to you.

Moreover, your profile and the personal data will be stored on Google Forms, and later Google Drive and other services operated by Google LLC, 1600 Amphitheatre Parkway, Mountain View, CA 94043, USA ("Google"). Google is certified for the US-European privacy agreement "Privacy Shield", which ensures compliance with the data protection level applicable in the EU. By sharing your information with us, you allow TokenWoman to share the information publicly on the website (e.g. in the form of a personal “profile”). The information we collect and share publicly includes: Name, Position, Company, City, Country, Company Expertise (two options), and whether you speak for free, expenses covered, or paid. Whatever social links you make available will be publicly shared; including your LinkedIn, Telegram, and Twitter. Your email address will not be shared unless you explicitly specify it.

The photo you provide will also be featured on the website. It will be to the discretion of TokenWoman to publish your profile online (for an unlimited period of time as the case may be), unless you inform us that you revoke your consent to publish your profile.

You have the right to request for your profile to be edited or removed at any time. You also have the right to  information and deletion of your personal data in accordance with data protection regulations. And you can revoke your declaration of consent at any time with the consequence that the processing of your personal data, in accordance with your declaration of revocation, by this becomes inadmissible for the future. This does not, however, affect the legality of the processing which took place on the basis of the consent up to the revocation
